RESPITE SERVICES
What is Respite Services?
The program is designed for caregivers of the chronically ill, so they can have some time away for a vacation or other breaks.
It is temporary care! The primary care giver is expected to resume care after the short stay
Note: You can stay no longer than 42 days in a 12 month period.
Who is eligible for this program?
- At least 16 years old
- Lives in Wayne County
- Needs 24 hour care/supervision
How is the program paid for?
- Active Medicaid (This is arranged through the Department of Social Services)
- Private Pay
What is included?
- Aide assistance-personal care, supervision, guidance and direction
- Personal Laundry
- Medication administration
- Meals and Snacks
- TV and phone
- Leisure Activities
- Medical management of health conditions
- Case management of needs
- Linens, supplies and equipment
What isn’t included?
- Medications
- Transportation
- Beautician/barber
- Therapy evaluations and services
- Medical services: Physician, dental, podiatry, radiology, lab
- Electrocardiogram
What Should I bring?
- Medications needed for the length of stay
- Clothing for 5 days
- Personal belongings (read material, puzzle books, etc)
How to apply?
- Application
- Medical assessment from primary care physician and medication orders
- TB skin test within 1 year
- PRI and Screen (nursing assessment) through home care agencies such as Wayne Community Nursing, Lifetime Care, or Visiting Nursing Service of the Finger Lakes
- Copy of insurance cards, power of attorney and Health Care Proxy
- Agreement for services
